Clinical trials for AD therapeutics have commonly used some version of the ADAS‐Cog at longitudinal visits to assess the efficacy of the treatment in slowing cognitive decline; for nearly two decades, trials of amyloid beta (Aβ)‐targeted therapies have failed to show efficacy, 28 although some have shown results in the ADAS‐Cog that were trending toward significance. 29 If language item scores are reliable and most heavily influenced by the progression of mild AD, the clinical trial data might be reassessed post hoc using language subdomain items, or specifically “comprehension of spoken language,” as most items from the other subdomains were less reliable, and their reliable information was more heavily related to interindividual trait differences.
