There were statistically significant differences by age ( P < 0.001), race/ethnicity ( P < 0.001), education ( P < 0.001), household income ( P < 0.001), and place of residence at time of study enrollment (i.e., urban, suburban, rural) ( P < 0.001) but not gender (although gender difference was borderline significant at P = 0.054) (see Web Table 1).
