The correlation between serum and tissue drug levels was significant when tissues were uninflamed but failed to reach significance in inflamed tissue. 24 Patients who are nonresponsive to anti-TNF therapy tend to have higher serum to tissue ratio of concentration, suggesting that tissue concentration may be the more predictive factor of response to therapy. 25 In the current study, only 36% of the patients were in endoscopic remission; therefore, many patients had significant inflammation which may account for the lack of an association between these variables.
