However, while nurses can improve postpartum education and assist with social determinants that impede access to care, structural challenges that nurses are not able to address, such as Medicaid eligibility policies, shortage of primary care practitioners, and lack of collaboration between obstetric and primary care practitioners, which may be significant barriers to improving transition of care. 5 , 33 In exploratory analysis, we found that the intervention led to a small but significant reduction in ED visits without admission in the first 12 weeks post partum, while there were no differences in other outcomes, such as hospitalizations and outpatient visits.
