Recently, there has been an increasing trend towards the use of reverse shoulder arthroplasty (RSA) in elderly patients with complex fractures, as shown by the 2021 UK National Joint Registry (NJR). 10 However, some studies report a four times greater odds of a postoperative complication compared to hemiarthroplasty (HA), 11 and no significant improvement in PROMs compared to non-operative treatment. 6 There is therefore clinical uncertainty regarding the effectiveness of RSA for 3 or 4-part PHFs in the elderly.
