9 , 10 With the influx of newer biologic and small molecule therapies, there has been an increasing trend towards combining medications for the management of refractory cases. 11 , 12 This modality appears to be effective for inducing remission of active disease and in patients with significant extra-intestinal manifestations or concomitant rheumatologic disease. 12 , 13 While there is limited data on the use of multiple biologics or JAKi in combination to treat patients with IBD, there is much less information available to guide the successful de-escalation of combination therapy once adequate response is achieved.
