A third dose of 1 g vancomycin was administered intravenously based on the following per the clinical perspective because (1) the concentration in the waste dialysate showed a decreasing trend; (2) vancomycin was effective against the infection; and (3) no signs of vancomycin toxicities were observed such as vancomycin flushing syndrome (VFS), fever, neutropenia, thrombocytopenia, nephrotoxicity, or ototoxicity, which are concentration‐dependent side effects [ 11 ].
