Unfortunately, surveys based on national registry databases [ 46 – 49 ] show that clinical practice is not evolving as generally recommended, probably also owing to several factors, including difficult AL multidisciplinary management in an emergency setting, unavailability of latest technology in peripheral hospitals, and an increasing trend towards defensive medicine by general surgeons during their duties.
