Odds ratios showed a non-linear dose-response relation for the two highest fourths of organisational score: 2.73 (95% confidence interval 1.43 to 5.19) for the third quarter and 4.63 (2.11 to 10.17) for the fourth quarter, both highly significant (fig 2 ).
