Data in general has shown an overall trend of improvement of equity in access and outcomes due to many health sector reforms and health systems interventions, such as ensuring that services are free at the point of delivery; engaging communities and civil society organizations in the planning and delivery of health services; and introduction of a decentralization agenda in the delivery of services ( 17 , 26 ).
