Patients in the intervention group reported greater confidence in coping with the illness ( p < 0.001) and reduced stress and anxiety related to the illness ( p < 0.001). Discussion Our study indicates that the transitional care programme had a positive trend towards reducing the rate of readmission at 30 days post-discharge, and improving patient satisfaction regarding care.
