Despite high inter-rater reliability, the difference in capsular contracture rate between prepectoral and subpectoral cohorts was unable to achieve significance for each evaluator; however, given the result of the combination group, we anticipate a trend toward significance with a larger sample.

There is an increasing trend towards direct-to-implant (DTI) reconstruction in many centers, where the permanent implant is placed at the time of mastectomy in a subpectoral position, with the breast soft tissue supported along the inferolateral pole with a biomaterial surgical mesh, such as acellular dermal matrix (ADM) or synthetic material 5 .
