Gabapentin (a centrally acting calcium channel blocker and anticonvulsant) appears to be a stand-alone in the category of agents with a highly significant effect. 31 , 33 The exact mechanism of action of gabapentin is not fully understood, but is suspected to be attributable to its multireceptor affinity, potentially altering dopamine, serotonin, and norepinephrine. 32 As to the dosing and frequency of gabapentin, there is some consensus suggesting 100 mg/day for CKD patients along with continued assessment of symptom experience and titration of the drug.
