Prior research in Australia, the United States, and Europe has shown an increasing trend of nonutilization of kidneys (from donors of the DBD as well as the DCDD pathways). 1 , 5 , 7 Reasons for the increase in kidney nonutilization are unclear, but this has been attributed to other unmeasured factors such as fear of donor-transmitted disease, 15 poor perfusion at the time of retrieval, and logistic reasons including the lack of beds and operating lists. 16 Our study suggests that donor kidney function and the duration of warm ischemia are the 2 key variables that influence clinical decision-making.
