9 However, 3 studies failed to show increased NKCC2 expression with SGLT2 inhibitor use but instead showed a trend toward decreased expression. 10 , 11 , 12 Additionally, this mechanism fails to explain their beneficial effects on magnesium homeostasis in patients without a reduction in fractional excretion of Mg 2+ , as we reported previously. 6 Hence, improved magnesium levels secondary to changes in luminal electrical potential provide an unsatisfactory or at least inadequate explanation.
