The percentage of schools offering a formal course within a geographic region was calculated to account for regional variation in school density and survey response rates ( Figure 1 ), and although there was a trend toward higher rate of such courses being offered in the Northeast and West Coast regions, this did not reach statistical significance ( p = .07).
