In the absence of validated predictive indices of therapeutic response to rescue therapies, one should proceed to colectomy if there is a negative trend in: clinical indices (comprised of vital signs, rectal bleeding and abdominal tenderness), biomarkers (rising CRP), colonic dilatation on abdominal imaging or endoscopic deterioration.
