In fully adjusted models, the TyG-BRI index in the ECDRAHC cohort showed marginal significance for ischemic stroke incidence (HR 1.13, 95% CI: 0.99–1.30, p = 0.069).

The positive association of TyG-obesity measures with ischemic stroke attenuated after sequential adjustment for potential mediating factors, aligning with findings from other studies on this attenuated trend [ 7 , 46 , 47 ], and in the fully adjusted models, the significant association between TyG-related indices and ischemic stroke disappeared in rural Chinese residents, with only TyG-BRI approaching significance and showing an approximately linear relationship.
