SIRI showed a trend towards reduction over time ( p = 0.078), which did not reach conventional statistical significance, and should therefore be interpreted cautiously.

Discussion In this real-world study, we investigated the long-term trajectory of systemic inflammatory indices in patients with moderate-to-severe plaque psoriasis treated with guselkumab over a period of up to 156 weeks, observing an overall trend toward reduction in several markers—particularly PLR, MLR, and SIRI—supporting the hypothesis that IL-23 inhibition exerts sustained systemic anti-inflammatory effects.
