The natural indirect effect mediated through antibiotic prophylaxis also did not reach statistical significance and suggested a possible, though imprecise, protective pathway (HR = 0.93; 95% CI 0.85–1.02; P = 0.132), accounting for approximately 21% of the observed attenuation in the total effect.
