The 2013 guideline suggested treating the ruptured aneurysm “as early as logistically and technically possible to reduce the risk of rebleeding; if possible, it should be aimed to intervene at least within 72 h after onset of first symptoms.” This was based on a small randomised trial, 34 which showed a trend towards better outcomes in patients operated within 3 days after the bleeding.
