Zhang et al observed that the association between CTI and CVD risk was more pronounced in individuals aged 45–60 years, in women, and in those without chronic comorbidities 15 In contrast, Sun et al found that although non-diabetic individuals with elevated CTI exhibited a markedly increased risk of CVD, this association failed to reach statistical significance in patients with diabetes mellitus 16 This discrepancy may be attributable to the presence of profound metabolic disturbances, microvascular dysfunction, hypertension, and obesity in diabetic patients, which act as strong competing or confounding risk factors and may partially obscure the independent predictive value of CTI as a single composite marker.
