The ORs and 95% CIs for ever-smokers with ≤23, 23–41 and ≥41 pack-years of smoking compared with never-smokers were 0.60 (0.36 to 0.98), 0.48 (0.28 to 0.79) and 0.36 (0.21 to 0.60), respectively, with a significant trend (P trend <0.001).
