Key Points Question Was an organized fecal immunochemical testing (FIT)–based colorectal cancer (CRC) screening program associated with lower CRC mortality in a population-based setting in the Basque Country, Spain, over a 21-year period? Findings In this cohort study of 438 134 total all-cause deaths among approximately 2.2 million individuals from 2004 to 2024, biennial FIT-based screening was associated with a 50% decline in age-standardized CRC mortality among adults aged 50 to 69 years, with a significant trend inflection observed 3 years after program initiation.
