Similar associations were seen in the 10–15 km (HR: 1.55, 95% CI: 0.74, 3.26; p = 0.25) and >15 km categories (HR: 1.48, 95% CI: 0.66, 3.33; p = 0.35), when compared to the less than 5 km distance category; however, these did not reach statistical significance.
