In a French nationwide healthcare database sample of 3853 breast cancer patients, radiotherapy-treated patients had 28 permanent pacemaker implantations compared with 13 expected cases, corresponding to a standardized incidence ratio of 2.18, although the direct comparison with non-irradiated breast cancer patients was only borderline significant [ 193 ].
While no significant association between PIK3CA mutations and recurrence-free interval was observed overall, a strong trend for impaired recurrence-free interval was noted in hormone receptor-positive breast cancers with PIK3CA mutations (adjusted HR 1.64), and a significantly detrimental prognostic impact was observed in hormone receptor-positive, HER2-negative patients treated with aromatase inhibitors alone (adjusted HR 4.44) but not in tamoxifen-treated patients, suggesting that estrogen deprivation may be ineffective in the presence of PIK3CA mutations [ 57 ].

High waist circumference (HR: 1.12, 95% CI: 1.04, 1.20), elevated cholesterol (HR: 1.07, 95% CI: 1.01, 1.13), and high blood pressure (HR: 1.11, 95% CI: 1.05, 1.17) were each significantly associated with higher risk of breast cancer among postmenopausal women, and there was a clear trend in increased risk with increased number of MetS components among postmenopausal women.
The result of another meta-analysis of observational studies [ 36 ], two of which overlap with the previous meta-analysis [ 30 ], indicated a borderline non-significant 11% increased risk of breast cancer in postmenopausal women with MetS in cohort studies, a two-fold increased risk in studies with other study designs, and a 52% increased risk overall.
