6 Not surprisingly, in light of the pre‐existing diagnosis of RARS, a missense somatic mutation of SF3B1 was identified in our patient, which is among the 15 most commonly mutated genes seen in myeloid disorders and clonal hematopoiesis of indeterminate significance and is particularly prevalent (>80%) in patients MDS with ringed sideroblasts. 7 Our patient presented with an acute coronary syndrome on the background of no cardiovascular risk factors except a history of low‐grade MDS requiring regular darbepoetin infusions.

In terms of cytological diagnosis, 13 (1.6%) samples were classified as nondiagnostic, 202 (24.6%) as benign, 147 (17.9%) as indeterminate (142 atypia of indeterminate significance/follicular lesion of indeterminate significance AUS/FLUS, and 5 follicular neoplasm/suspicious for follicular neoplasm FN/SFN), 92 (11.2%) as suspicious for malignancy (SUSP), and 368 (44.8%) as malignant.

Limitations The current study has some notable limitations, first and foremost among which is our small sample size, possibly explaining the indeterminate significance of some findings.

Experts spoke of overdiagnosis as being of indeterminate significance because of uncertainty about the overdiagnosis rate.

Regarding the latter, the increasing use of older donors and the more likely presence of gene mutations associated with clonal hematopoiesis of indeterminate significance (CHIP) in older populations is an emerging concern ( Nawas et al. 2021 ).

However, the usefulness of FNAC is limited when dealing with follicular lesions presenting cellular atypia of indeterminate significance (Thy3), for which the distinction between benign and malignant lesions is ambiguous [ 11 ].

Fine-needle aspiration cytology of the cervical thyroid gland resulted in the lesion being classified as of “indeterminate significance,” and a pathological examination of a needle biopsy sample from the same site led to the lesion being diagnosed as a follicular neoplasm.

Appendix Supplementary Methods: Panel Design and Processing The panel is generally ordered for one of the following conditions: acute myeloid leukemia (AML), myelodysplastic syndrome (MDS), cytopenia, clonal cytopenia of undetermined significance, clonal hematopoiesis of indeterminate significance, myeloproliferative neoplasm, or other myeloid disorders.

A cervical biopsy was performed if women had abnormal cytology results (e.g., atypical squamous cells of indeterminate significance or more severe lesions), positive HPV results, or abnormal results from colposcopy.

The area under the glucose curve was lower in caffeine-treated subjects, but with indeterminate significance [ 19 , 22 , 24 ].

But, when nuclear grooves with oval shape, chromatin clearing, and nuclear overlapping, are seen in BTLs, they do cause a diagnostic dilemma, resulting in the diagnosis of “atypical cytology/indeterminate significance” on fine needle aspiration (FNA) samples [ 3 , 18 , 19 ].

The use of fine needle aspiration of a thyroid mass can be considered; however presence of cytology of indeterminate significance including follicular proliferation and atypia of undetermined significance in between 5 and 20% of cases limits the use of this method [ 2 ].

We combined cartilage grades 0 and 1, since a grade 1 lesion is of indeterminate significance and there were very few participants with grade 1 cartilage defect.
