6 Not surprisingly, in light of the pre‐existing diagnosis of RARS, a missense somatic mutation of SF3B1 was identified in our patient, which is among the 15 most commonly mutated genes seen in myeloid disorders and clonal hematopoiesis of indeterminate significance and is particularly prevalent (>80%) in patients MDS with ringed sideroblasts. 7 Our patient presented with an acute coronary syndrome on the background of no cardiovascular risk factors except a history of low‐grade MDS requiring regular darbepoetin infusions.
In terms of cytological diagnosis, 13 (1.6%) samples were classified as nondiagnostic, 202 (24.6%) as benign, 147 (17.9%) as indeterminate (142 atypia of indeterminate significance/follicular lesion of indeterminate significance AUS/FLUS, and 5 follicular neoplasm/suspicious for follicular neoplasm FN/SFN), 92 (11.2%) as suspicious for malignancy (SUSP), and 368 (44.8%) as malignant.

Lung nodules of indeterminate significance are discovered in over 25% of the individuals screened with CT. 32 As revealed by expensive and invasive diagnostic tests used to discriminate these lesions, 96% of these hard to distinguish nodules are benign. 33 The development of a simple, rapid, specific, noninvasive, and clinically relevant diagnostic tool would improve early lung cancer detection, reduce the need for unnecessary biopsies, and enable early treatment.
Regarding the latter, the increasing use of older donors and the more likely presence of gene mutations associated with clonal hematopoiesis of indeterminate significance (CHIP) in older populations is an emerging concern ( Nawas et al. 2021 ).
